An accident that resulted in a man being crushed to death in 2005 led WorkSafeBC to levy its highest penalty last year against EnCana Corporation. The investigation that led to a $150,000 fine concluded that a first-aid attendant wasn’t on site after the accident occurred and employees weren’t properly trained. As the prime contractor, EnCana, ‘‘failed to do everything reasonably practicable to co-ordinate activities . . . and ensure compliance,’’ WorkSafeBC said in a statement. The mishap occurred after EnCana hired Grande Prairie, Alta.-based brush-cutting company R-Test Contracting to cut a sightline through a forested area about 25 kilometres southwest of Dawson Creek. The owner of the company was crushed by improperly fallen trees while ‘‘performing work duties that were significantly beyond his abilities,’’ the incident report said.
